An exciting opportunity has arisen for a Band 4 Assistant Practitioner (Nursing Support Worker) based in the Llanelli Locality District Nursing Team.
The role will allow you to support the management of a delegated caseload within the Community Nursing Team. The role will provide high quality and complex care management to patients in their own homes, with the guidance and support of an experienced District Nurse Team Leader and community staff nurses.
The post holder will ensure high standards of care are delivered to patients and their families/carers within the home or clinic environment. This includes liaising with staff, patient/carers and other agencies/organisations as required to ensure appropriate delegation of duties and risk management, whilst ensuring effective communication between those involved in the care package.

We are looking for dedicated, enthusiastic, proactive professionals, who are highly motivated and caring.
This is a permanent part time position working 30 hours per week over a 7-day period including weekends and bank holidays. The post holder will need the ability to travel between sites in a timely manner.
